3 السالم عليكم ورحمة ه للا وبركاته جميع ا.. المحاضرة الماضية كانت Bacterial Genetics واآلن راح نحكي عن Group of Gram negative bacteria اللي هي بتضم: 1.Haemophilus 2.Bordetella 3.pseudomonas aeruginosa (Next Lecture). 1,2&3 are covered in these lectures bc they share 2 significant features of structure & physiology: 1. They all have gram negative cell, therefore they contain lipopolysaccharide which is a very ( بأول دقيقة ما بينت (كلمة maybe he said (rare factors) :) 2. They grow in the presence of oxygen, and therefore they cause infections at site where oxygen tension is high (in lung and other vital tissues) >>so they cause respiratory diseases. these include here those that are primarily/ exclusively pathogens of human respiratory tract ( Haemophilus & Bordettela) (Haemophilus & Bordettela) infect only & mainly Respiratory system. (Pseudomonas Aeruginosa) can infects wide variety of tissues and mainly in immune compromised patients. In addition to lung infections, it causes another infections ( in next lecture). حكى الدكتور عن المرجع اللي بالصورة مع أرقام الصفحات للي بيحب يرجع.. مش مطلوب طبعا منا الكتاب فقط الساليدات وتسجيل المحاضرة كافي لهذه المرحلة ال : Haemophilus & Bordettela 1. زي ما احنا شايفين شكلها بالصور ألنها صغيرة ما بتكون rod coccus shape وال rod shape وإنما بتكون جاية بين ال shape وصغيرة. 2. وهي فقط موجودة بال.Humans 3. أهم نوع من الHaemophilus هي اللي بسميها influenzae اللي ما بتسبب لنا اإلنفلونزا ألنه اإلنفلونزا العادية سببها..viral infection لكن هذا النوع من البكتيريا بيسبب acute purulent meningitis.mainly وراح نحكي عن أمراض أخرى بتسببها ال.Haemophilus 4. ال Bordettela اللي راح نهتم فيها اسمها Bordettela pertussis اللي بتسبب لنا ال pertussis= whooping cough اللي هو السعال الديكي اللي كان زمان منتشر. BORDETTELA 1. ال إضافة. 2 تحتاج ل oxygen tension 3. وبالتالي هي موجودة بال.respiratory system 4 ال. إضافة 5. لها 7 أنواع راح نهتم بأول نوع B.pertussis that causes Whooping cough.1.(pertussis)disease. (most common).b.parapertussis that causes mild pertussis-like illness.2 B.pertussis + B.parapertussis" are the human pathogens ".of this genus 1 الفريق العلمي TEAM SCIENTIFIC

4 point 1: because it causes Whooping cough which is highly contagious disease (point 3) and the significant cause of morbidity & mortality worldwide, 51 million cases.and >600,000 deaths each year point 2: The major mode of transmission of Bordettela is via droplet, spread by coughing, so it is airborne droplet transmitted disease :point 3 ال إضافة Bordettela pertussis binds to ciliated epithelium in the upper respiratory tract, the bacteria produces a variety of toxins and other virulence factors that interferes with cellular activity, eventually causing death of these cells. هال راح نحكي هاي ال mechanism شو اللي بيصير و شو هم ال virulence factors produced by Bordettela purtssis in order to cause infection/colonization in the upper.respiratory tract Virulence factors: A.Those which helps bacteria in cell Adhesion it uses this to produce an attachment ( we know that the first step of any infection is the Attachment): 1.FHA 2.Pertactin 3.Agglutinogen these all three that play a role in facilitating the Attachment, and Anchore Bordettela to the respiratory epithelial cells والدكتور أشر على الصورتين أنه هيها هيك. B. Virulence factors help in producing bacteria Toxins there are some toxins produced by this type of bacteria, and.one of these toxins is Trachial ytotoxin (TCT) النقطة األولى عنه كما هي ال إضافة النقطة الثانية وبسبب األولى فبالتالي بتعمل لنا paralysis of the cilli الموجودة على ال respiratory epithelium وبالتالي بيصير mucous.trigger a violent cough reflex to clear airway و اللي build up وبالتالي راح يتراكم ال mucous في ال respiratory epithelial cell وبصير عند هذا المريض cough reflex يعني عدد كبير من موجات ال cough حتى يحاول التخلص من هذا ال.mucous واألسطر تحت بالساليد بتحكي عن: Bordettela pertussis does not directly invade the cell of respiratory tract or spread to deeper tissue site وإنما بتعمل damaging and distraction في هذه ال respiratory epithelium وما بتعمل invasion داخلها. الفريق العلمي TEAM SCIENTIFIC 2

5 The second virulence factor(toxin) in toxins field is Pertussis Toxin (PTX) that produced by B.Pertusses It has 3 Functions: point 1: slides.. means improving the attachment of B.pertussis to the respiratory epithelium. point 2: so, if there is a B.pertussis infected patient who has Whooping cough, if we take his blood sample, we will find increased lymphocytes number in(his complete blood count CPC) and this is bc PTX اوك هو بسبب هاد ال ptx عمل زيادة في عدد ال lymphocytes اللي هي خاليا مناعية وفيها t cells ممتاز لكن هاي الزيادة عالفاضي لل lymphocytes بالدم ألنه PTX بيمنعها تغادر الدم و ما راح يوصلوا لمكان ال infection في ال respiratory epithelium وما راح تؤثر مناعي ا بخاليا البكتيريا كونها ما وصلتها. point 3: histamine causes vasodilitation so the fluid leaves the blood and enters to the tissue (airway tissue inside lung), with then airway swelling decreasing the diameter of the airway making it hard to breathe, this makes Whooping expiration The third virulence factor(toxin) in toxins field that produced by B.pertussis is Adenylate Cyclase Toxin (CYA) Functions: point 1: invasive toxin means that it damages and destructs respiratory epithelial cells point 2:--- point 3:--- point 4: : تعني أنه قرأها كما هي يعني من الساليد وال إضافات. ال Pathogenesis تاع ال B.pertussis راح يدخل على ال respiratory epithelium وراح يصير لها Attachment وبعد هيك راح يصير destroy ciliated يعني cilia بال damaging& distraction cough وبالتالي mucous build up وبالتالي بيصير عنا epithelial cell و حتى persistent coughing بنوبات محاولة من المريض أنه يعمل.respiratory epithelium الموجود داخل ال mucous لهذا ال clearing 3 الفريق العلمي TEAM SCIENTIFIC

6 The incubation period for pertussis ranges from 1 to 3 weeks The disease can be decided into 3 phases another name is healing state (recovery).3 وراح نحكي عنهم بالتفصيل. بكرر ال )---( يعني نفس اللي بالساليد دون إضافة. stage.1 Cararrhal المرحلة األولى زي الرشح العاد ي يعني. This phase begins with relatively non specific symptoms such as rhinorrhea,mild fever, dry cough,mild conjunctival infection, malaise, sneezing, runny nose, nasal congestion, red&watery eyes,.. so it is flu-like symptoms in this phase عن آخر نقطة بالساليد ; في هاي المرحلة يبدأ معدي بشكل كبير ألنه بيكون في عنا زيادة عدد للبكتيريا الموجودة في ال.respiratory epithelium 2.Paroxysmal stage المرحلة الثانية weeks 1-6 في هاي المرحلة بيصير worsening of the cough ويظهر ال Whooping cough اللي تعريفها تكملته بالساليد الصفير بالنهاية بيظهر عند أخذ ال..respiration اللي بنسميه زي السعال الديكي مميز بصوته. نقطة 2 برضو حكى بفضل تشوفوها على اليوتيوب لسماع الصوت وهو سهل التمييز. نقطة : 3 day up to 50 times per نقطة : كما هي نقطة 5: هسا عملية ال continuous coughing أو اللي هي very aggressive ممكن تعمل complications عند المريض فممكن أنه يصير عنده : vomiting, bc it especially infects infants & kids.collapsed lung...tiny Hernias الفتق سواء كبير أو صغير بالعمر remains extreme fatigue because this cough that طول اليوم. 4 الفريق العلمي TEAM SCIENTIFIC

7 ا حكى السطرين اللي باألول طبعا المضاعفات تشمل النقاط بالساليد كالعادة وأنا بس بكتب اللي بيضيف الدكتور شرح عليه.. الرجاء االنتباه لكافة المكتوب بالساليدات : ا Pneumonia ألنه بيصير عندي superinfecting ألنه صار في & damaging distraction بال cilia االي هو بتمثل جزء من ال immunity فهيك سيء وبالتالي ممكن يصير عنده (pneumonia).secondary Bacterial infection ا Dehydration هو حتى ما بيقدر ألنه ياكل ويشرب من عملية ال coughing المستمرة + loss weight equal oxygenation in blood ألنه راح تقل عملية ال seizures بكرر النظر للساليدات هو المعتمد طبعا. المكتوب مني هو اإلضافات والشرح فقط. الرجاء االنتباه stage. Covalescent. 3 المرحلة الثالثة بالنهاية بعد أسابيع ألشهر بيصير عنا عملية ال recovery وال healing بال.airway بعض الكتب بتحكي هم أصال بس ال stages 2 اللي قبل فوق. عملية ال diagnosis of B.pertussis غالبا تعتمد على ال pertussis is يعني مثال.clinical picture of patient maybe suspected in an individual who has catarrhal symptoms اللي عنده هاي ال flue-like symptoms within 1-3 weeks فبعدها بيبدأ ال 2) Whooping sounds of cough (stage فبالتالي ممكن أنه.suspected بس هاد في حال الزم يكون مخالط لشخص مصاب بال.Whooping cough هسا ممكن نعمل gram stain ويطلع عنا طبعا Gram negative.. لكن ال definitive diagnosis هو ال cultures و في عنا ال selective culture media لل B.pertussis اللي هو اسمه Regan-Lowe agar..culture is #1 ### حكى الدكتور ما في داعي نعرف األشياء الباقية في تكملة الساليد. ال culture طبعا تستخدم لكن ألنه احنا بحاجة ل rapid diagnosis مرات فممكن نستخدم ال mucous / للبكتيريا اللي داخل ال DNA&RNA بأنه نشوف ال molecular study respiratory epithelium عن طريق تطبيق ال..PCR or ELISA وهاي ال 3 نقاط كلها تنظر Look for antigenes/antibodies /RNA/DNA تاعت البكتيريا وبالتالي ممكن نحدد وجودها أو ال...وهدول # 2 و 3 و 4 في حال بدنا أسرع 5 الفريق العلمي TEAM SCIENTIFIC

8 هو ال Erythromycin is the drug for infection with B.pertussis ك : as a chemotherapy [treatment therapy] and - as chemo prophylaxis for the contact with this - patient فهم نعرف إنه سواء ال erythromycin كان للمريض أو للمخالط أنه نعطيه prophylaxis وقاية للمخالطين. ال 3 أمثلة هاي good choice for B.pertussis النقطة 2 بتحكي عن ال pertussis vaccine it is available and has a significant effect in the lowering the incidence of.whooping cough هام جدا و حاليا ما بتشوف حاالت Whooping cough بسبب وجوده. يحتوي هذا ال vaccine على sporulated و protein purified from B.pertussis in combination with (Diphtheria & Tetanus toxoid و (Pertussis بنسميه DTP غالبا عنا باألردن بيبلشوا األطفال أخذه على عمر شهرين >>وهاد كان اللي بدنا نعرفه عن هذه البكتيريا HAEMOPHILUS The most common & important is Haemophilus influenzae وراح نحكي شوي عن اللي تحتها.ducreyi point 1,2,3 كما هم كانوا يعتقدوا زمان أنه هاي ال Haemophilus influenzae هي اللي بتسبب اإلنفلونزا ولهيك اسمها هيك فالتسمية أصال خاطئة. H ducreyi point 1 هاي المطلوبة وهاد فقط اللي بدنا نعرفه عنها وما قرأ نقاط 2 3 Haemophilus Influenzae The major human pathogen of this genus are pleomorphic, ranging from coccobacilli to long, slender filaments. H.influenzae may produce a capsule (six capsular types have been identified). capsule is an important virulence factor for this bacterial cells different types are a,b,..,f H. influenzae disease is associated particularly with capsular type (b) which(capsule) is composed of polyribose phosphate H.influenzae type b is especially important as pathogen of young children, although it can cause disease in individual of all age group so, H.influenzae has a polysaccharide capsule that has different types a-f and 95%...slides etc. 6 الفريق العلمي TEAM SCIENTIFIC

9 حكينا تسمية كلمة influenzae اوك خاطئة لكن تسمية Haemophilus صحيحة وجاية من heme +philuse و philuse=love بما معنى أنه هي بتحب الدم.. فقرأ الدكتور الساليد هون x factor = factor 10 * V factor = factor 5* فهاد النوع من البكتيريا حتى نعمل لها culture فبنحتاج ال RBC وهي ما بتقدر تاخد ال factors 2 من ال RBC فالزم نعطيها ال RBC/ damaged.h influenzae وبالتالي بتنمو ال lysis نتيجة ال factors 2 بحيث بيطلع لنا ال. Chocolate agar وهاي هي طريقة عمل ال...RBC lysis by heat في هذا ال media بينماا إذا حطيناها في blood agar مااا راح يكون في.growth point 1: Transmitted via respiratory droplets in.coughing & sneezing situations point 2:... non tybable= not from a-f so it doesn't cause infection /disease ( weak virulence) :point 3 ما قرأها point 4: because it os part of our normal flora point 5: 1/200 especially child dies by age of 5 (patients infected by H.influenzae especially type b) شوف أهمية ال vaccination في عملية reduce the incidence of this type of disease caused by H. influenzae type b كان عدد كبير ونزل لحتى أنه بال 2004 بطلنا نشوف هذا النوع من المرض invasive disease اللي بسببها. virulence factors produced by this type of bacteria in order to colonize the respiratory epithelium making infection. FIRST there is polysaccharide >capsule< especially in type b, it prevents bacteria phagocytosis SECOND because it is gram negative there is lipopolysaccharid and has anti-phagosytic effect 5 THIRD it has enzymes, neuraminidase & IgA protease (so it can destroy immunoglobine antibody in respiratory epithelium tissue) FOURTH it has lipooligosaccharide LOS, it is like lipopolysaccharide but with a shorter sugar, and it is toxic to cilli in respiratory epithelium. from FIRST -to FOURTH, they all follow endotoxins, and there is no exotoxins produced by this bacterial cell type 7 الفريق العلمي TEAM SCIENTIFIC

10 we said that H.influenzae is a part of our normal flora, so it is found in our oral cavity & nasal cavity, most of it as non tybable H.influenzae, but it is possible that there is type b from it as a part of our normal flora, so the infection can be happened by two ways 1.Local invasion صار عنا مثال over growth لها أو أنه ال immunity of the patient قلت فأدى ذلك أنه هاي الجزء اللي من our normal flora يصير لها local locally وهو التهاب الجيوب األنفية فهو انتقال sinusitis & وهو التهاب األذن الوسطى otitis media وتعمل لنا invasion 2.Systemic invasion وأيضا ممكن هاي البكتيريا الموجودة as a part of our normal flora وصار لها ability to get into the blood stream making Bacteremia وتروح على ال distance organs زي لو تعمل لنا meningitis أو تعمل infection in joints or bones زي ما حكينا أنه ال clinical presentation مختلفة H.influenzae causes many different types of diseases يعتبر ال pneumonia من ال local invasion ألنها كانت هاي موجودة بال oral and nasal cavities و وصلت لل lung وممكن حكينا تعمل systemic وتروح تسبب meningitis عال meninges و بتعمل Osteomyelitis cellulitis, Arthritis بالعظام.. طبعا هاد كله كان قبل ال vaccination. more than 50% of the patients that diagnosed with meningitis are caused by the H.influenzae before vaccination. وهاد النوع من ال meningitis that caused by H.influenzae type b can be very severe و غالبا بيصير عند المريض complications زي ال Hearing impairment or neurological وعنا أيضا mortality of this type of infection هاد كله كان قبل ال vaccination فبطلنا نشوف this type of meningitis اللي هي بتسببها حتى لو شفنا بعض الحاالت اللي بسبب H.influenzae type b بتكون غالبا mild أو أنها.diagnosed early and can be treated 8 الفريق العلمي TEAM SCIENTIFIC

11 laboratory diagnosis and identification, usually a definitive,diagnosis generally requires identification of the organism وبالتالي نحتاج )اوك صح بنستخدم عملية gram stain بداية( لكن mainly نحتاج to.isolate the microorganism using a culture media وفي حالة H.influenzae نحتاج إلى chocolate agar نحتاج supplementation with factor 10 and factor 5(NAD) in order to be cultured in chocolate agar وبالتالي هاي ال factors اللي بنحصل عليه عن عملية lysis لخاليا الدم بيسمح لهاي البكتيريا بالنمو.. وبالتالي this bacterial cell cannot be grown in blood agar يعني إذا عنا chocolate agar و blood agar ولقينا growth على ال chocolate وما لقينا growth عال blood فبنعرف أنه هاي.H.influenzae إذا عملنا cavities, isolation of H. influenzae from oral or nasal which H.influenzae here is considered as a part of our normal flora فهذا ما بيعطينا.indication لكن إذا عملنا isolation for this bacteria from CSF or let say joints فإنه هاد بيعطينا indication أنه في عنا.infection فمش كل isolation of H.influenzae يعتبر.pathogenic.. ال IsoVitaleX-enriched chocolate agar هي اللي بستخدمها. طبعا ال biochemical tests for H.influenzae هي اللي بدنا نعرفها بأمثلتها هذول ال 4 catalase, oxidase, nitrate reduction and glucose fermentation أما ال tests الثانية ما حكاها. When invasive H.influenzae is suspected, use suitable antibiotic, for example 3rd generation cephalosporin such as *ceftriaxone or.. should be started as.soon as possible تأكدوا من اللي حكاها هون مش واضحة حتى من النت دقيقة 38:20 so Hospitalization is required for the patient in case of an invasive disease such as meningitis caused by this type of bacteria Antibiotic sensitivity testing is necessary because there is emergence of strain resistant to commonly used antibiotics to treat H.influenzae )emergence of resistant strain to cephalosporins( -we can use Chloramphenicol plus ampicillin too as a good choice for treatment.so those were in an invasive disease cases. -But In mild like sinusitis, otitis media and other upper respiratory tract infection, this could be treated with "Trimethoprimsulfamethoxazole" /or/ ampicillin plus amoclan{plus clavulanic acid}. -Prevention, active immunization[vaccination] against H.influenzae type b is effective in preventing invasive disease and decreasing respiratory colonization of H. influenzae.يعني ما بتالقي عنده Type b as a part of normal flora يعطيكم العافية SOME RULES MAY CHANGE 9 الفريق العلمي TEAM SCIENTIFIC
