٣- إذا اكن عالج املريض باستخدام األقراص املنمظة للسكر األخرى : - اإذا كان الدواء املتناول على جرعة واحدة قبل شهر رم ضان يجب اأخذ نف س اجلرعة مع وجبة الإفطار. - اإذا كان الدواء املتناول على جرعتني قبل شهر رم ضان يجب أاخذ اجلرعة الأوىل مع وجبة الإفطار ون صف اجلرعة الثانية مع وجبة ال سحور. ما هي األمور التي يتوجب على مريض السكري الذي يعالج باألنسولين االلتزام بها أثناء شهر رمضان )يشمل السكري من النوع األول والثاني( - يجب اأخذ نف س اجلرعة من الأن سولني طويل الأمد مثل لنتو س ليفيمري مع وجبة الإفطار. - يجب اأخذ جرعتني فقط من الأن سولني ق صري الأمد مثل ا سبارت اجلرعة الأوىل مع وجبة الإفطار والأخرى مع وجبة ال سحور. - يجب اأخذ اجلرعة الأوىل من الأن سولني املخلوط مع وجبة الإفطار والثانية مع وجبة ال سحور. - يف ضل تخفي ض جرعة الأن سولني وقت ال سحور اإىل الن صف يف بداية شهر رم ضان تفاديا لحتمال انخفا ض ال سكر. ما هي األمور التي يتوجب على مريض السكري الذي يعالج بالحبوب المنظمة للسكر واألنسولين مع ا االلتزام بها أثناء شهر رمضان يجب عليه اتباع الن صائح اخلا صة بكل نوع من العلج كما هو مو ضح اأعله. من هم المرضى األكثر عرضة لمشاكل ارتفاع وانخفاض مستوى السكر أثناء الصيام قد ي شكل الصيام خطورة بالغة على حياة بع ض الأ شخا ص لذا يلزم مناق شة اإمكانية ال صوم مع الطبيب املخت ص قبل الإقدام على ال صيام كما يجب اللتزام باملراقبة الدقيقة ملستوى السكر بالدم حتت إا شراف طبي كامل وميكن اإجمال هوؤلء الأ شخا ص يف الآتي: - مر ضى ال سكري الذين يعاجلون بالأن سولني خا صة بجرعات كبرية. - الأ شخا ص ذوي ال سكر غري منتظم قبل شهر رم ضان. - امل صابون بالهبوط املتكرر يف م ستوى ال سكر قبل شهر رم ضان وخ صو صا هبوط اللوعي. - من تكرث اإ صابتهم بحمو ضة الدم ب سبب ارتفاع م ستوى ال سكر. - احلوامل. متى يلزم كسر الصيام عند الإح سا س باأعرا ض النخفا ض اأو عندما يكون م ستوى ال سكر اأقل من 60 ملغم يف أاي وقت اأثناء ال صيام.

What should diabetic type II patient do during the month of Ramadan? 1- If the patient is just on diet therapy - Continue the diet plan and follow up with dietitian. - Test the blood sugar at the initial days of fasting once or twice a day, and then only when needed. 2- If the patient on a pill (Glucophage) - Take the medication in only two doses, the first one at Eftar and the other one at the time of Al Sahoor. - Patient may need to take only half dose with Al Sahoor particularly at the beginning of the month of Ramadan. 3- If the patient uses other tablets: - If the medication taken as a single dose before the month of Ramadan, the dose should be taken as it is with Eftar meal. - If the medication is taken as two doses before the month of Ramadan, the first dose should be taken with Eftar meal and half of the second dose should be taken at Al Sahoor meal. What should insulin user patients do during the month of Ramadan (including diabetes type I and II)? - The same dose of long term insulin such as Lantus and Levemir should be taken with Eftar meal. - Two doses of short term insulin such as Aspart should be taken, the first dose with Eftar meal and the other one with Al Sahoor meal. - First dose of the insulin mixture should be taken with Eftar meal and the second one with Al Sahoor meal. - It is prefer to decrease the insulin dose to the half in Al Sahoor at the beginning of the month of Ramadan in order to avoid the possibility of low blood sugar level.

What a diabetic patient should do? - Get ready for the month of Ramadan one month in advance. - Discuss with physician all necessary change of doses and timing of medication. - Discuss fasting diet program with dietician and diabetics educator. - Avoid hard physical exertion during fasting. - Drink a lot of liquids during Eftar period to avoid dehydration. - Monitor blood sugar accurately, particularly at the initial days of fasting at the beginning of the month of Ramadan at least three times a day. Testing blood sugar to be done in the following times: before Eftar meal, before Suhoor meal and in the middle of the fasting and Eftar periods. - Adjusting the medication doses may be needed several times during the month of Ramadan.
